1. You hereby grant your consent to Symphera s.r.o., with registered office: U Potoka 26, 252 65 Tursko, Praha – Západ, ID No.: 28361296, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, Insert 136102 (hereinafter referred to as the “Controller”), to the following within the meaning of Regulation (EU) No. 2016/679 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and on the repeal of Directive 95/46/EC (General Data Protection Regulation )(hereinafter referred to as the “Regulation”) processed your personal data provided by you in the context of completing the questionnaire sent by the company.
  2. In the event that a special category of personal data is processed in the context of the responses to the questionnaire, you hereby grant the company your express consent to such processing in accordance with Art.9 para.2 lit. (a) Regulation.
  3. The personal data provided in responses to the questionnaire may be processed on the basis of your consent and will be processed for the purpose of evaluating the responses from the questionnaire so that the services provided to the Controller’s customers can be improved. This data will be processed by the Controller for a period of 2 years.
  4. You give your express consent to processing as described above. The provision of personal data is voluntary.
  5. Consent can be withdrawn at any time, for example by sending an email or letter to the company’s contact details at info@agilerace.cz
  6. Please note that under the Regulation, you have the right to:

    • withdraw consent at any time,
    • request information from us about what personal data we process about you,
    • request access to   to have this data updated or corrected, or to request a restriction on processing,
    • require us to delete such personal data, unless it is personal data that we are obliged or authorised to further process under applicable law,
